Rosie O’Donnell is coming undone about some things she fixed behind the scenes.

The comedian is sharing a major secret on her Substack, revealing that, like every normal person, she also harbors some insecurities, which she addressed by going under the knife.

Rosie O’Donnell had earlier expressed regret about her past use of Botox, describing it as a huge mistake, although her dramatic weight loss transformation came with super impressive results.

In a painfully introspective poem, “Decisions,” posted on her Substack on Monday, May 25, the 64-year-old comedian confessed to a sense of guilt and shame over her cosmetic surgery; a “secret facelift.”

The facelift has come in the car’s price at what O’Donnell described as a privilege level that she has never experienced in the past.

The actress also admitted she used to think facelifts were a betrayal of feminism and will proudly claim to be one of the women who would never get their hands dirty under the knife.

After dropping 50 pounds and facing the physical changes in her look, O’Donnell admitted that her view about the world and beauty had changed.

At first, she was going to accept the aging of nature, but she confessed that it began to feel like lying when she did.

O’Donnell raved about losing 50 pounds after being prescribed the Type 2 diabetes medication Mounjaro in January 2023.

The comedian explained that her main reason for starting the medication was to manage her Type 2 diabetes. At the same time, the dramatic weight loss ended up becoming an added benefit of the treatment.

O’Donnell revealed that her 13-year-old, Clay, dissuaded her from getting a facelift, saying her wrinkles were a result of her life, and that there were a lot of young women who liked her for the way she got older.

The talk had a profound impact on O’Donnell, as Clay’s views echoed the ones she once held. The actress said that the comments made her delay the procedure for several months, giving much thought to the decision.

Ultimately, O’Donnell concluded that it was important for her child to learn that they can make choices about how they look and how their body looks.

The former talk show host now says she’s happy with the results, noting that the facelift didn’t affect who she is, just how she looks.

The process was so subtle that most people don’t really notice it, said O’Donnell, and she now says she no longer has to fight against the reflection.

As for the future, the actress said that she will keep sharing her truthful story whenever she feels like it; it’s a way for her to embrace this next chapter of her life.

As noted by The Hollywood Reporter, the comedian discussed the years that followed her hugely successful daytime talk show, which concluded in 2002.

According to her, her exit meant the casting directors did not have the chance to cast her as someone different on screen because of her fame and personality.

As she grew older, O’Donnell stated that she had always envisioned herself in the kind of roles that legendary actresses such as Geraldine Page and Colleen Dewhurst had inhabited, a path of mature, character-driven performances.

For this reason, she never intended to change her look through cosmetic surgery and would rather age in a way that would align with her projections.

The comedian pointed out that in the wake of their personal beliefs and activism, it may be difficult for the entertainment industry to see her as a neutral performer who can slip into a character.

She noted that her criticism of the Iraq War and former president George W. Bush also made her a polarizing figure in the public eye, with some audiences taking her side and others strongly opposed to her opinion.

Yet, O’Donnell raved that going out of the spotlight also enabled her to take time out to care for her children and recuperate from the intense fame she had during her talk show days.

Last year, PEOPLE reported that the talk show host got candid about a cosmetic treatment she had during her time hosting “The Rosie Show” on the Oprah Winfrey Network. The actress admitted that she felt like she had made a huge mistake right after getting the injections, as she could not get past the feeling of her face being taped down.

O’Donnell emphasized that she does not recommend for anyone take such a risk, although that is not her first time undergoing a major procedure. In 2013, the actress underwent a Vertical Sleeve Gastrectomy surgery, which helped control her appetite in a bid to help her lose weight.

Over a decade later, the comedian opened up about using a weight loss medication that came with impressive results she flaunted online.

Despite recording all these successes, O’Donnell revealed on Instagram that she was still haunted by severe body dysmorphia, often making her feel like she was not doing enough.

As stated by The Blast, the media personality was back in the trenches of expressing her concerns over Donald Trump and the state of the nation.

In an interview with MSNBC host Nicolle Wallace on her podcast, “The Best People,” O’Donnell admitted that even her therapist wondered why she is still so emotionally involved in Trump’s presidency.

During the course of the discussion, the actress asserted to Wallace that the full effects of Trump’s leadership have yet to be felt and cautioned the nation that a serious crisis could be in store for America if it does nothing.

In a bid to escape Trump’s presidency, O’Donnell moved to Ireland in January 2025 and continues to maintain her anti-cruelty, anti-political crusade from overseas.

The actress added that she has kept the fire burning because she believes Americans should stand up for constitutional values and against cruelty in politics.

Meanwhile, O’Donnell admitted that she fully understands the emotional toll the political environment is taking. The star urged Americans to do what they can to take care of their mental health without allowing the spew of chaos that has engulfed Trump’s administration to overwhelm them.

Rosie O’Donnell is really taking her open-book policy seriously!
