Lil Nas X is in the hospital after a shocking late-night incident in Los Angeles that ended with the rapper being restrained by police and rushed to receive medical care.

The “Old Town Road” hitmaker was spotted on Ventura Boulevard around 4 a.m. Thursday morning, August 21, wandering the streets in nothing but underwear and cowboy boots. The Grammy winner appeared disoriented as he muttered about attending a party.

At one point, Lil Nas X placed a large orange traffic cone on his head while pacing along the empty street.

Witnesses who saw the unusual behavior quickly alerted authorities, fearing it may have been a medical emergency. Several calls were placed to the LAPD, with reports suggesting a possible overdose.

When officers arrived, Lil Nas X was still wandering in the middle of the street. According to TMZ, things reportedly escalated when the rapper allegedly charged at officers, prompting them to subdue him and place him in handcuffs for his own safety.

Paramedics on the scene determined it may have been a possible overdose and transported the “Panini” artist to a nearby hospital for treatment. While he is currently being treated, sources say he could still face charges related to the incident.

The shocking scene has left fans stunned, as this isn’t the first time Lil Nas X has been tied to drug use.

In a 2020 cover interview with Variety, the rapper opened up about a “scary” period in his life when he turned to marijuana to cope with loss and the pressures of fame.

He explained that the deaths of hip-hop stars like Nipsey Hussle, XXXTentacion, and Juice Wrld, along with the passing of his grandmother, left him devastated and anxious.

“So much happened during my rise. You know, drugs and murders,” he said at the time, adding that his grandmother’s death in 2018 was especially hard. “It was devastating. And it made me a hypochondriac: I’d wake up, heart racing. It was scary.”

Lil Nas X admitted that he began self-medicating during that period, “smoking [weed] heavily” as a way to cope. Eventually, he said he began to feel “more connected with the universe” and tried to take those experiences as signs to help him move forward.

Interestingly, Lil Nas X has also made headlines for his comments on drug use with fans.

During his “Long Live Montero” tour in 2022, the rapper urged concertgoers to stop bringing one particular substance to his shows.

“Stop doing poppers at my concert! U do not need ur a–hole relaxed to see me perform ‘Industry Baby!’” he tweeted on September 28, sparking a wave of viral reactions online.

Just days later, he doubled down with a tongue-in-cheek clarification. “Aht aht. I said stop doing poppers. Cuz ya a–hole don’t need to be open while I sing my depression album cuts,” he wrote, before adding, “If y’all wanna do weed, molly, shrooms, paint, gasoline ect. knock yourselves tf out.”

This latest incident isn’t the first time Lil Nas X has faced a health scare in 2025.

Back in April, the rapper was hospitalized with partial facial paralysis, sparking major concern among fans after he posted a video from his hospital bed. In the clip, the right side of his face appeared frozen as he explained that he was working hard on recovery.

“I’ve been chewing a lot so I can get this muscle stronger,” he said at the time, pointing to the affected side of his face while wearing a pink shower cap. He assured followers he was improving, adding, “It’s much better, it’s much better. My eye still has to play catch-up. But like, I can give a genuine smile, so that’s good. I’m still winkin’ at mothaf-ckers, but… yeah.”

Now, just months later, news of his possible overdose has once again put the 26-year-old’s health in the spotlight, raising new concerns about his wellbeing.
